Welcome to Unlimited Pathways to Care, where the needs of our clients are the priority! We plan to work with you or your loved ones to ensure basic needs can be met with the care and compassion they deserve. To be eligible for PCS:
- Age birth through 20 and have Medicaid.
- Must have an ongoing disability, physical or mental illness that has been confirmed by a doctor or practitioner.
- Have a Practitioner Statement of Need signed by a practitioner (physician, advanced practice nurse, or physician assistant) who has examined you in the last twelve months.
- Need help with ADLs and IADLs based on the Personal Care Assessment Form.
- Give a reason why your responsible adult cannot help you with ADLs and IADLs.
Compassionate Care Services
Our Home Care Aides will assist with all needs while providing a fun companion. Let the caregivers at Unlimited Pathways To Care help make life easier. We provide, but are not limited to, many of the same treatments as a live-in care facility. We Offer the Following Services: Personalized care to assist with activities of daily living (ADLs), such as:
- Bathing
- Eating
- Assisting with toileting
- Dressing
- Assisting with walking
We also assist in instrumental activities of daily living (IADLs):
- Laundry
- Light housework
- Meal preparation
